The identity of the man in the fatal incident at Kaynak Recycling was determined

Oct. 28—LIMA — A federal agency continues to investigate an incident that led to the death of an employee at Resource Recycling, Inc. on Tuesday, Oct. 22.

The U.S. Department of Labor’s Occupational Safety and Health Administration launched an investigation following the death of employee Miguel A. Martinez Serna, 50, at the facility at 1596 Neubrecht Road, Lima.

Martinez was working on a conveyor approximately 12 feet above the ground when the conveyor was started. Scott Allen, Regional Director for public affairs and media relations for the U.S. Department of Labor, said Martinez fell off the conveyor and fell to the ground, suffering fatal injuries.

OSHA has six months to complete its inspection, issue citations and recommend fines if violations are found during the inspection process.

Martinez, of Waynesfield, was born in Hidalgo, Nuevo Leon, Mexico, before coming to Lima for work decades ago, according to his obituary. She leaves behind her mother, Rebecca, and her sons, Brisa Martinez of Spencerville and Miguel Angel Martinez Jr. of Mount Vernon. He left two children named. He enjoyed watching football, playing golf, fishing and cooking, according to his obituary. He traveled at least once a year to Hidalgo, where his mother still lived, and often called her every day, according to his obituary.

Resource Recycling, Inc. did not respond to a request for comment on the situation as of Monday morning.

In July, the company experienced a mulch fire that lasted for several days. No injuries were reported in this incident.
