Davenport Fire Department ‘Life File’, free medical history card for area residents

DAVENPORT, Iowa (KWQC) – The Davenport Fire Department shared on Facebook that it now provides medical history cards called the “Life File.”

Medical history cards are stored in a red, magnetized pouch that can be stuck to something like a refrigerator, according to the Davenport Fire Department. This then allows first responders to access the patient’s medical history in the event of an emergency.

It was stated that the form contains information about the patient’s allergies, medications, emergency contact persons and more authorities.

The Davenport Fire Department said the cards and pouches will be offered free of charge to all Davenport residents.

To request, visit the City of Davenport website Here or contact the Fire Prevention Bureau at 563-326-7906.
